What Happens and What to Bring
Preparing for Your First Visit
- Please request any relevant medical records from previous doctor testing or treatments, and have them mailed or faxed to the CT Fertility office which you will be visiting.
- Request or download our New Patient Questionnaire. Complete all items, and return to the office you will be using.
- Feel free to call or email a CT Fertility Financial Counselor with any questions you may have regarding insurance, payment, or pre-certification.
Your First Visit
Please bring the above items to your first visit with Dr. Doyle. At that time, he will:
- Review your complete history.
- Construct a comprehensive testing plan.
- Formulate and discuss treatment options.
