Making Babies

If you are interested in using a sperm donor for either artificial insemination or in vitro fertilization, you must complete these simple steps:

I. Physician Consultation: Arrange for a consultation with your CFA physician who will request all appropriate medical screening, including bloodwork (Blood Type, HIV, Hepatitis B screening) and anatomical studies. The possible use of fertility medications will also be reviewed at that time.
II. Choose donor: Select your donor choices from the available donor list.
III. Contact CFA Sperm Donor Coordinator: All patients planning to pursue sperm donation must first meet with CFA's Sperm Lab Manager, Lynn Kurjiaka, to schedule the required screening test results; and to choose the ideal sperm donor.
IV.  Complete paperwork: Review and complete all relevant consent forms regarding sperm donation.
