Screening Requirements for Intended Fathers / Sperm Donors

Initial Interview
During this first visit, you and your partner (if applicable) will meet with Dr. Doyle and/or other members of CFA´s medical and nursing team to review your specific goals; describe all aspects of your testing and treatment cycle; discuss the IVF and surrogacy processes in detail; and review any questions or concerns which you may have. At that time we will assist you in completing the required FDA Screening Questionnaire and appropriate consent forms will be provided.

Physical Examination
We will need to perform a physical examination within six months of you treatment cycle. If you do not have this performed at CFA, then your physician must provide us with a completed Physical Examination Form which we will provide you.

Bloodwork
The required FDA Screening Bloodwork ensures that essentially no risk of serious disease transmission to your carrier is present. These tests include HIV-1/HIV-2, HTLV-I/II, Hepatitis B Surface Antigen, Hepatitis C Antibody, CMV, RPR, and urethral cultures. In addition, genetic counseling and testing is encouraged if you have any concerns about your specific family history , or questions about your donor's history.
PLEASE NOTE: This FDA-required bloodwork MUST be performed ONLY in an FDA-approved laboratory. Therefore, if you do not have your bloodwork drawn by us and intend to use a local laboratory, you MUST confirm with us that the lab which you plan to use meets FDA standards, and allow us to place the request order.Also, this bloodwork must be performed TWICE -- FIRST as a screening test WITHIN SIX MONTHS of your treatment cycle; and AGAIN WITHIN SEVEN DAYS of the donor's egg retrieval and your sperm donation.
